I had appointed you to exercise certain powers under the general/special power of attorney authenticated by the Notary Public in [Notarial location], under No. [Power of attorney number] / [Year] / [Reference], dated [Day]/[Month]/[Year].
I have now decided to terminate this power of attorney, dismiss you from it, and cancel the aforementioned instrument. Accordingly, I hereby notify you to cease representing me under it as of the date on which you are served with this notice, failing which any use of it after that date shall be deemed fraud and breach of trust.
I further request that you provide me with the outcome of the acts carried out in execution of the power of attorney, submit an account thereof supported by the evidentiary documents, and deliver to me the funds you received on my behalf under it, together with their interest and proceeds, within [Number of days] days from the aforementioned date, at my place of residence located at the address stated above.
If you delay doing so, I reserve my right to bring civil and criminal proceedings against you in respect thereof and for the offense of breach of trust, in addition to seeking compensation for every harm suffered by me, including all loss and loss of profit.
